This form is intended to provide a standard means of recording observed or reported faults or disputes, and of preserving relevant data in cases where further investigation is needed.

A person must leave a class 4 venue if required to do so by, or on behalf of, the holder of a class 4 venue licence
The holder of a class 4 venue licence, or any person acting on behalf of the licence holder, need not give any reason for denying entry to a person or requiring a person to leave a venue (Section 307, of the Gambling Act 2003)

Once a Venue Manager has identified a person who he or she has reasonable grounds to believe is a problem gambler, a private meeting must take place to provide advice and discuss any issues.

Self Exclusion
A Venue Manager must issue an exclusion order prohibiting th person from entering the gambling area for up to 2 years if the person has identified themselves as being a problem gambler and has requested that the venue prohibited them from entering the venue concerned. (Section310, of the Gambling Act 2003)

Venue Exclusion
A Venue Manager may, after offering advice or information to a person [who he or she has reasonable grounds to believe is a problem gambler], issue an exclusion order to the person that prohibits the person from entering the gambling area of the class 4 venue for a period of up to 2 years. (Section 309, of the Gambling Act 2003)
